Your insurance

To apply for insurance cover, or increase your existing cover, you need to complete this form.

You will also need to complete AIA Australia’s Full Personal Statement if you are 55 or older and are:

  • applying for more than $1 million of Death or Death & TPD cover
  • applying for more than $8,000 per month in Income Protection cover.

If you need to claim on your Income Protection or Total and Permanent Disablement insurance, call us on 1800 757 607. We’ll send you the necessary form and talk you through the process.

Benefit payments

If you wish to make a partial withdrawal of existing unrestricted non-preserved monies from your account, complete this form. You will also need to provide certified copies of your ID documents, if they haven’t been provided to Maritime Super in the last five years.

Please call us on 1800 757 607 if you would like to move the money in your Maritime Super account to another fund. We will talk you through the process and provide you with the necessary form.

If you have permanently moved from Australia to New Zealand and would like to transfer your super to a New Zealand KiwiSaver Scheme you will need to complete this form.

Waiver forms

Complete this form if you are a Contributory Accumulation member with a minimum protected benefit and are cashing, transferring or splitting your contributions prior to retirement to confirm that you understand that you will waive your right to the minimum protected benefit now and in the future.
